INSTRUCTIONS: Enter attendee name and affiliation as you would like them to appear on your name badge, as well as your preferred email address, which will be used for meeting- and UMRCC-related correspondences only, and will not be shared with 3rd parties. Then, complete all required fields and enter the quantity of each registration type to be purchased. Confirm order total near the bottom of form and after clicking “Submit” button, you will be taken to a separate and secure Stripe page for completing payment by credit card (MIGHT TAKE UP TO A MINUTE FOR PAYMENT PAGE TO APPEAR – PLEASE DONT CONTINUE TO CLICK SUBMIT DURING THAT TIME). If paying by method other than credit card , submit the form below to register each attendee, save a copy of the Stripe payment page(s) and contact Levi Solomon (firstname.lastname@example.org).
